OU Health benefits are designed to support your overall wellbeing, adapt to your evolving needs and provide cost savings and convenience when using OU Health (OUH) Custom Network – ensuring your family’s health is protected every step of the way.
- Medical: Choose from three medical plans through BlueCross BlueShield: PPO, HDHP 1 or HDHP 2. Each plan offers access to the OU Health Custom Network, as well as in-network physicians, pharmacies and hospitals. Using OU Health providers can lead to substantial savings.
- Employees who work from home outside of Oklahoma will have access to a separate high-deductible plan.
- Dental: Select from three dental coverage options through Delta Dental: PPO Choice Advantage, PPO Basic or PPO Plus Premier Premium.
- Vision: Choose between two vision plans through the VSP Choice Network: Basic Plan or Enhanced Plan.
Understanding Your 2026 Medical Plan Choices
OU Health continues to find innovative ways to help you manage rising healthcare costs. You can save significantly by using the OU Health Custom Network and by participating in the Wellbeing360 savings program.
Below is an overview of medical plan coverage, including paycheck premiums and treatment costs for the OU Health Custom Network, in-network and out-of-network providers.


Employees who fulfill the requirements for Wellbeing360 savings will receive a $25 reduction in medical premium rates per paycheck.

*Generally, you pay the full cost of covered services until you meet the annual deductible. After that, you and the plan share costs through copays and/or coinsurance.
** PPO Plan Family Coverage: Each family member has an individual deductible. Once a member meets their deductible, the plan begins paying its share for that individual. When the combined total reaches the family deductible limit, the plan begins paying its share for all family members.
*** HDHP 1 and HDHP 2 Family Coverage: The family deductible applies collectively. As family members receive services, their expenses count toward the total. Once the combined total reaches the family deductible limit, the plan begins paying its share.
**** Out-of-Pocket Maximum: The annual out-of-pocket maximum is the most you’ll pay for covered services, including medical, prescription drug and behavioral health, within the plan year. With family coverage under all three plans, each member has an individual out-of-pocket maximum. Once a member reaches the limit, the plan pays 100% of the member’s covered expenses. When the combined total reaches the family out-of-pocket maximum, the plan pays 100% of covered expenses for all family members.

HOW OU HEALTH SUPPORTS YOUR SAVINGS
OU Health offers several tax-advantage accounts — managed by Optum Financial — designed to help you maximize your savings and plan for both expected and unexpected healthcare expenses. These accounts give you control over
your healthcare dollars while reducing your tax burden.
CHOOSE THE ACCOUNT THAT FITS YOUR NEEDS:
- Health Savings Account (HSA): Available if you enroll in a HDHP. Enjoy triple tax advantages — pre-tax contributions, tax-free growth and tax-free withdrawals for healthcare expenses. Your HSA balance rolls over year after year, building long-term savings. Your HSA is fully portable and belongs to you. You can take it with you if you change health plans, jobs or retire.
- Flexible Spending Account (FSA): Set aside pre-tax funds for eligible healthcare expenses throughout the year. What you save on taxes stays in your budget for other priorities.
- Limited Purpose FSA (LPFSA): Available to HDHP participants for dental and vision expenses only. Maximize your savings by pairing this with your HSA.
- Dependent Care FSA (DCFSA): Use pre-tax dollars for child and/or elder care expenses while you work. This helps you balance your family responsibilities while reducing your tax burden. The IRS sets annual contribution limits. For 2026, the maximum is $3,750 for individuals and $5,000 for families. (Highly compensated employees may be ineligible or limited to participate in this program.)

The above chart compares and contrasts key features of the accounts available with OU Health’s medical plan options. Keep in mind, you can enroll in FSA even if you do not enroll in OU Health medical, dental or vision coverage.
The tax-advantage accounts are governed by Internal Revenue Service (IRS) rules, regulations and guidelines, which are subject to change by the IRS.
*Exception: Up to $640 of any funds that remain in your FSA or LPFSA as of the deadline for submitting 2026 reimbursement claims (March 31, 2027) will be rolled over into a 2027 FSA for you. Any amount above $640 will be forfeited.
HEALTH SAVINGS ACCOUNTS (HSA)

- An HSA is an individually owned, tax-free (federal) savings account used to pay for eligible health expenses, now or in retirement. It’s one of the most powerful tools for long-term financial wellbeing, with triple tax advantages.
- OU Health invests in your future: We contribute to your HSA quarterly. One-fourth of the annual company contribution is deposited with the first paycheck of each quarter, real money to boost your health and financial security.
- Your money, your future: The money in your HSA is always yours to keep and rolls over year after year. You can take your unused balance with you when you leave or retire, building a safety net for healthcare expenses throughout your life.
- Catch-up contributions for added security: If you turn 55 in 2026 or earlier, you’re eligible to make an additional “catch-up” contribution to accelerate your savings as you approach retirement.

Upon enrolling, you will receive a debit card from Optum Financial. To access your contributed funds, you must activate the card before use.

OU Health offers three dental plans through Delta Dental of Oklahoma, giving you flexibility to choose coverage that fits your needs and budget. While all three plans cover the same services, premiums and coverage levels vary.



*Employees in the PPO Choice Advantage dental plan will receive a schedule of coverage that lists the pricing and copays per service. Benefits shown are for in-network providers and are based on negotiated fees. Out-of-network coverage is based on reasonable and customary (R&C) charges.
DELTA DENTAL’S HOW® PROGRAM: PERSONALIZED CARE FOR YOUR ORAL HEALTH
Delta Dental’s HOW® Program (Health through Oral Wellness®): Your oral health needs are unique, and this innovative program offers enhanced benefits for individuals at higher risk of cavities or periodontal disease. Giving you the extra support you need to protect your teeth and overall health. After your dental office completes a simple HOW assessment, you may qualify for additional annual benefits designed specifically for you, including:

OU Health offers vision coverage through VSP with two flexible options and access to a network of quality providers. LONG-TERM DISABILITY (LTD): PROTECT YOUR INCOME AND YOUR FUTURE
An unexpected illness or injury shouldn’t derail your financial stability. OU Health offers long-term disability insurance options to full-time employees to help protect your income and maintain your quality of life if you’re unable to work.
- Full-time employees may elect LTD insurance coverage of 50%, 60% or 66 2/3% of their base annual salary.
- Part-time employees are not eligible to elect LTD insurance.
- LTD benefits begin after the elimination period has been met.
LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT (AD&D): SECURITY FOR YOUR LOVED ONES:
BASIC LIFE AND AD&D COVERAGE
OU Health provides benefit-eligible employees with basic life insurance equal to 1.5* times their annual base salary at no cost to you. This coverage gives you immediate financial protection for your family.
Eligible employees will be auto-enrolled.
*Two (2) times base annual salary for director level and above.
SUPPLEMENTAL LIFE INSURANCE COVERAGE
For additional peace of mind, OU Health offers supplemental life insurance options for you and your dependents on an after-tax basis. You pay the full cost of elected supplemental coverage.
Employee Coverage:
- Full-time employees may elect additional life insurance coverage up to five times their base annual salary, with a maximum coverage of $6,000,000.
- Part-time employees may elect a flat $15,000 in coverage.
- Age-based benefit reductions begin at age 65.
Dependent Coverage:
Child(ren): Coverage up to $10,000.
- For full-time employees.
- Children six months or younger, are limited to $5,000.
- Part-time employees may elect up to $5,000.
Spouse:
- Coverage up to 50% of the employee’s life insurance amount, with a maximum of $75,000.
- If the spouse is age 70 or older, the maximum coverage is $10,000.
- For part-time employees, the maximum benefit is $7,500.
SUPPLEMENTAL AD&D COVERAGE
Full-time employees may elect additional AD&D insurance coverage up to five times their base annual salary. AD&D benefits are paid if you become seriously injured or physically disabled due to an accident. Depending on the type of loss, the benefit amount ranges from 50% to 100% of the elected coverage. These benefits are paid in addition to any life insurance coverage.
For all Life & AD&D policies, reduction in coverage begins at age 65.
UNDERSTANDING EVIDENCE OF INSURABILITY (EOI)
Employees who elect supplemental insurance coverage exceeding one times their base salary may be required to submit an EOI. An EOI is information requested by the insurance company about your past and current health to verify whether you meet the definition of good health. This is a standard process to help you access the additional coverage you need.

HOW IT WORKS
Rates: Premiums for additional voluntary benefits are based on your age and/or the coverage amount you elect. You’ll see your personalized rates in Workday during the enrollment process, making it easy to choose the coverage that fits your budget.
When You Can Enroll: Enrollment in these plans is available:
- Within 30 days of your hire date.
- During annual open enrollment.
- Within 30 days of a qualified life event.
Cost: These voluntary cash benefits are paid for by you through after-tax premiums deducted from your paycheck. You have full control over the coverage you choose and the investment you make in your financial security.
